You will need to complete a registration form (GMS1) and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

You can also register online https://gp-registration.nhs.uk/G82799. Before you are accepted as a patient at the practice you will need to sign all the appropriate forms. We will then be in contact with you to confirm you are registered with us.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

You can obtain a registration pack from reception.

Named GP

Every patient that is registered with Sheppey Healthy Living Centre will be allocated a Named GP. This process will be completed for new patients when they register. It will not always be possible to see your allocated named GP.

Guide to GP Services
